Thread-eye Diagrams

First make a bar tack the width of the hook plus about 4 threads.

If you 'bury' the thread when you start, coming up in place with no 'tail' showing, you can have a very clean loop.
Starting at one end of the tack, use buttonhole stitch (aka blanket stitch) to cover the surface threads of the tack.
Pack those buttonhole stitches on there well and make sure you cover the tack entirely and bury the thread to finish.

This leaves a thread-covered loop that's fairly wear-resistant.
Pretty easy, eh?
